[daughter]’s Birth Certificate and Social Security Card

Please forward [daughter]’s birth certificate and social security card to be as soon as possible.  Being that I have sole parental authority over [daughter]’s well being and affairs, I require these documents to enroll her in activities, open accounts for her etc.  I also, apparently, have had an incorrect social security number for her all of these years and need to update her insurance records with [insurance company].  They require a copy of her social security card to update her file.

In addition, she needs her birth certificate for summer camp and the latest date that I can submit it will be June 18, 2010 or else she will be unable to be enrolled in Summer Camp this year.

Please advise when I can expect these documents in the mail.

Just to confirm our conversation of this morning as you failed to respond to my request for her social security number or the fact that I stated it was urgent and could not wait for a request through the state of [third state] (which was done already.)

Me: Please send me [daughter]’s social security card and birth certificate ASAP. Third request. Thanks 9:05 AM

[The Ex] [last name]: Please get from the state or ss office. This is the first request I have gotten. You have all the authorization to get the originals. Thanks. 9:09 AM

Me: I have received nothing from you nor your attorney regarding anything of substance. Requests of this nature must be done in writing and/or through [Custodial Dad’s other state lawyer]. 10:22 AM

Me: I do not have her accurate social security number, please provide. I was hoping to have you forward me [daughter]’s birth certificate so that I can enroll her in summer camp. No time to wait for a state request. Forms need to be finalized June 18 or she will be unable to attend. 10:24 AMI understand your reluctance to send me the originals however, I am desperate for her birth certificate as I need it in less than 2 weeks to enroll her in summer camp.  I have already requested a copy of her certificate however, I can not wait until they send it to me.  If you send it, I can send it back to you after enrollment.

If you can fax me her social security card, that would suffice for insurance purposes and I will be able to obtain a new card and update [insurance company] with that as well.  Again, I do not have an accurate social security number for [daughter] and [insurance company] does require the card in order to make chnages in her records.  Please advise on when you intend to fax and I can give you our home fax number.

Time is of the essence and I would greatly appreciate a response to this before the end of the week.
